Advancing excellence in health care requires voluntary acceptance of valid, standardized practices that minimize patient risk and optimize clinical outcomes, improve quality of care, reduce malpractice risk, reduce cost of care and comply with regulating bodies. Evidence rather than opinion should guide clinical decision-making with standardized instruments, data collection protocols, analyses of test results and reporting. Clinically practical application of a standardized approach to assessment and targeted treatment will be illustrated using a case-based approach. Formulation of treatment goals and association of scores to overall dysphagia severity will be introduced. Tracking swallowing outcomes (physiologic, intake status, quality of life) over time and standardized reporting using electronic, web-based methods are demonstrated.
